Please feel free to contact the nurse during school hours of 7:30 AM and 2:15 PM with any questions or concerns.  Please notify the school nurse if your child is unable to attend school.  Messages may be left on the private nurse line and voicemails will be checked throughout the day. 

COVID-19 Protocol:
  • Children and staff who test positive must isolate for at least 5 days (day 0 is test date/symptom onset). They may return to school on day 6 and wear a mask through day 10.
  • If the individual has a negative test on Day 5 or later, they do not need to mask.
  • If the individual is unable to mask, they may return to programming with a negative test on Day 5 or later.

General MA Immunization and Physical Exam Requirements:
The school nurse is responsible for ensuring that all students have documentation of all MA school requirements. Please provide updated Physical Exam and Immunization records to the health office Documents can be mailed, dropped off, emailed to kventosi@holbrook.k12.ma.us, or faxed to 781-815-1627.  


Immunization requirement for 7th grade:
All incoming 7th graders will need to provide documentation of a Tdap and a meningococcal conjugate vaccine. Please consult with your child's pediatrician to ensure these have been given and provide the school nurse with documentation.  Click here for more information on this new requirement. 

Immunization requirement for 12th grade:
All incoming 12th graders will  need to provide documentation of having 1 booster dose of meningococcal conjugate vaccine (Men ACWY) received on or after 16 years of age. Please consult with your child's pediatrician to ensure these have been given and provide the school nurse with documentation.  Click here for more information on the new requirement. 

Medications:
If your HMHS student will require medication during the school day, you can call the school and make an appointment to visit with the nurse.  The nurse phone number is 781-767-4616 ext 3
     Please remember to bring:
1. The medication in its original prescription bottle
2. A medication order from the MD or NP. It may also be faxed to 781-815-1627. 
3. Yourself!  A parent or guardian needs to sign the parental consent form
