School Enrollment Requirements
MASSACHUSETTS STATE GUIDELINES FOR SCHOOL ENTRANCE
Immunizations
Massachusetts State Law requires that students be fully immunized against DTap, Polio, Measles, Mumps, Rubella (MMR), Hepatitis B, and Varicella. See new student health office requirement link below for immunization requirements. The statute includes exemptions for medical and religious reasons.
Physical Examinations
Physical examinations by a licensed physician, nurse practitioner, or physician assistant are required for all new students entering any grade. These exams must be completed within one year prior to starting school. Students in grades 7 and 10 are also required to have updated physical examinations.
To participate in any interscholastic sport, students must have documentation of a physical exam conducted within the past 13 months by their primary health care provider. This documentation must be submitted to the school nurse before tryouts.
A Massachusetts School Health Record is kept on file for every student.
